How Relationships Heal the Trauma of Combat

Research now explains how the trauma of combat effects the brains of our troops. The part of the brain effected by combat impairs the person's ability to sooth the brain's alarm system, making combat veterans vulnerable to suicide or violent behavior

I once had a patient who was a Vietnam combat veteran. Upon discharge he went from jungle combat to his home in 48 hours. Day after day he waited on his porch surrounded with his loaded rifles and handguns. One day he walked into town and held up a local bank at gunpoint. Prior to his combat experience, he was a responsible citizen. What happened to him?

The answer can be found by understanding how severe trauma affects our brain. So without sounding too much like a biology lesson, here goes.

When we experience aggression, fear and rage the part of the brain called the amygdala lights up. This is the fight part of the fight or flight survival circuitry. What keeps the amygdala in line and quiet is the hippocampus. This is a resource for memory and making sense of the trials and joys that life dishes out to us. It tells the amygdala that things are OK to calm down.

In Dr. Louis Cozolino's book, "The Neuroscience of Psychotherapy," he documents how the hippocampus is actually damaged by stress and trauma. The hippocampus of people exposed to combat trauma is half the size than before they were traumatized. So, the amygdala is left unchecked with the result being increased irritability, insomnia, aggression and self-destructive tendencies. People having experienced the trauma of sexual or physical abuse in childhood also have diminished cellular mass in the hippocampus.

The other brain circuit associated with trauma and stress is called the HPA axis. This is the brain's connection with the survival response of the human organism where the entire system becomes mobilized to protect itself from mortal threat. When the HPA axis is fired off, a biochemicals are released that actually destroy brain and cardiac cells to quickly produce energy to survive. It protects the organism much like a desperate person might burn the furniture to keep from freezing to death, according to Dr. Cozolino.

These chemicals are called glucocorticoids. Post mortem autopsiesdone on men killed in the Korean War, showed severe coronary artery damage. This damage may be evidence of the corrosive effect caused by sustained glucocorticoid release.

The nature of this war, with it's reliance on reservists and it's disconnection from the day to day lives of Americans, will amplify the isolation of returning troops and the severity of trauma symptoms. Soldiers who experienced early childhood loss and trauma would be more vulnerable

The behavioral medicine to restore and regenerate the hippocampus is to make sense of trauma within the context of trusting relationships. Connecting with others, especially with those who have experienced similar challenges, and together attempting to find meaning in trauma, is powerful healing.

With the future arrival home of thousands of troops who experienced severe combat trauma, more research and education is crucial. Focus needs to be directed toward understanding how their systems have been affected and to develop improved treatments including: medicine, brain nutrition and psychotherapy.

Welcoming home our troops with banners or parties needs to be paralleled with a serious, more informed and comprehensive response to the unseen effects of combat trauma.
